An Ethical People Ontology Plan with population health benefits

To harness the full benefits of genomic medicine requires the development of human phenotype ontology, to serve as a standardised, evolving language for describing individual biological profiles. The UK’s ambition to become a global leader in health innovation hinges on the ethical and equitable deployment of advanced technologies. This article outlines an ‘Ethical People Ontology Plan’ proposal in a foundational ecosystem for a new era of population health management (PHM). Forged from an alignment of four national strategic plans: the Genomics Medical Service Draft Specifications (Genomics England Plan); the Life Science Sector Plan; the NHS 10-Year Plan; and a new People Plan - this initiative uses an agentic AI-driven ecosystem to create biological Pre-Post eXams. 

These phases enable a fundamental shift from reactive to proactive care, powered by real-world data and federated quantum learning. By focusing on human phenotype ontology (HPO), this plan operationalises a clinical decision support (CDS) ecosystem that not only personalises medicine but stewards public trust, mitigates health inequalities, and cultivates a society prepared for the future of genomic health. Ultimately, the Ethical People Ontology Plan serves as the human-centric backbone for technological advancement, ensuring that a robust ethical framework, a skilled workforce, and an engaged public are central to achieving population health benefits and a healthier, more equitable future for the UK.
