Cardiac troponins and the diagnosis of heart disease: some articles in the current literature

Cardiac troponin T (cTnT) and troponin I (cTnI) are cardiac regulatory proteins that control the calcium-mediated interaction between actin and myosin. Raised cardiac troponin concentrations are now accepted as the standard biochemical marker for the diagnosis of myocardial infarction.

Possible mechanisms behind cardiac troponin elevations

Hammarsten O, Mair J, Möckel M, Lindahl B, Jaffe AS. Biomarkers 2018 Jul 6: 1–26. doi: 10.1080/1354750X.2018.1490969 (Epub ahead of print).

Cardiac-specific troponins are elevated in blood following cardiac injury and are the preferred diagnostic biomarkers when acute myocardial infarction is suspected clinically. Cardiac troponin elevations are also observed in clinical conditions without obvious connection to cardiac injury. Irrespective of the underlying condition, cardiac troponin (cTn) elevation is linked to a poor prognosis, even if the elevation is stable over time.
